Instructions:
Special Incident Report - This form is to be used to report a special incident that is not covered by a Caution Summary Report, Dismissal Report, or a Referee Assault Form (e.g. abandoned game, reporting misconduct by a Coach, outside interference by spectators etc.)
Referee Assault Form - In any case involving physical contact with a match official, the Referee Assault Form should be used.
A separate form must be completed for each incident.
Caution Reports - To be filled out and submitted only in the event of cautions against team officials. Caution reports do not need to be filled out for routine player cautions.

Tournament pay will be based on the length of tournament games. As an example, if a U15 Tournament game is 2x30 min halves, which represents 75% of a full-length game, the referee will be paid 75% of the U15 full-length fee, which is $41.25 in the case of a U15 game. Note: clubs may round up (but not down) to $45/Ref and $30/AR. |
